Q What is orthognathic surgery?

A It is a surgery improving the oral maxillofacial deformity, which is generally called orthognathic surgery that surgically operates on deformed oral and maxillofacial. It is well known to the public as a surgery correcting lantern jaw or distorted facies. However, it not only corrects distorted facies, but also it improves the esthetics through the harmony of jaw bone through spatial changes of jaw bone for skeleton malocclusion that is beyond the treatment that could give satisfactory occlusion of set of teeth, and helps to rearrange the set of teeth and improves the function of oral and maxillofacial.
Such surgery dramatically improves 3 dimensional esthetics, and oral maxillofacial function, helps the prognosis, and shortens the entire period of treatment taken to achieve treatment goal, but it cannot solve details of individual tooth problem, gives huge stress to patient and guardian regarding surgical risk, contains risk of general and local anesthesia, and has financial burden from hospitalization and surgical treatment.

Q What are the complications of tooth extraction?

A 1. Bleeding : After extraction, patient must keep nipping on gauze for 2 hours. If patient has high blood pressure, diabetes or other liver disorder, one must nip on gauze for more than 2 hours, and if necessary, patient will be discharged after bleeding stops. The cause of bleeding after extraction is mainly infection, or general disease mentioned above. If there is bleeding on extraction site, patient must nip on disinfected gauze for 1 hour, and visit nearby dentist or ER for continuous bleeding.
2. Edema : After extraction, the inflammatory edema is frequently observed, and it continues for 3 days after extraction and decreases afterward. The excessive size of edema occurs due to periostitis of soft tissue during extraction, and edema due to inflammatory.
3. Dry socket : Due to smoking or other causes, the blood clot in extraction wounds cannot be recovered normally, and frequently degenerated inside extraction wounds. Its major symptom is severe pain and bad breath. If such symptom is observed, patient must see doctor for treatment.
4. Infection after extraction : Infection after extraction is mostly gone after 2 days, but if swelling and pain continues, swelling in cheek and neck increases, or trismus occurs with fever , it is highly possible that there’s infection, which requires consulting with dentist.

Q Should we extract wisdom teeth?

A  Wisdom teeth is also called third molar, and it is erupted around age 20.One of the most frequent questions is whether to extract wisdom teeth. It is still a controversy. However, as wisdom teeth often cause disease, it requires immediate extraction. For example, as wisdom teeth are too hard to be reached by tooth brush and cannot be cleaned well, it is easy to have cavity, and infect the gum. This could cause pain and bad breath, which requires extraction. Also, if wisdom teeth are lying on gum, it might stimulate second molar and cause absorption, or could cause inflammatory due to food between wisdom teeth and second molar. The infection could cause damage in alveolar bone, and if it is left alone, it could cause osteomyelitis. Thus, if wisdom teeth are erupted, it must be consulted with dentist and requires extraction or consistent management. 

Q Relationship between general disease and dental treatment

A In general, people think general disease and dentistry has no connection. However, the influence of entire body problem to dental procedure is diverse, and the correct apprehension on problem is a very useful information in determining the safely planned dental treatment. The most common case is high blood pressure, which means arterial tension of adult in relaxed condition higher than 140/90mmHg. It is not prohibited to operate oral and maxillofacial surgery on moderate high blood pressure patients under local anesthesia, but the surgery must be delayed for severe high blood pressure patient(systolic blood pressure to 200mmHg or over, diastolic blood pressure over 110mmHg). There is also diabetes. Diabetic patients tend to have delayed recovery of scar due to changes in blood vessel and have high infectious rate. Other than these diseases, patients must accurately inform the dentist with their diseases such as heart disease, kidney disease, pregnancy and tuberculosis. And if necessary, the dentist must consult to specialists of the regarded disease for safer and accurate surgery.

Q What is oral and maxillofacial surgery?

A It is the department that is unfamiliar to the public. Maxillofacial refers to jaw and face, thus, oral and maxillofacial surgery is a field of surgery based on dentistry treating patients with surgical approach on innate, and acquired disease, trauma and damages in mouth, jaw and face.
